Medically Fragile Adult Waiver (MFAW)
Medically Fragile Adult Waiver (MFAW) program enables individuals with serious and complex health care needs to remain safely in their homes with their families by providing medically necessary services.
Types of Services
Medically Fragile Adult Waiver program services may include one or more of the following types of services:
- In-Home Personal Care
- In-Home Advanced Personal Care
Qualifications
Qualifications for the Medically Fragile Adult Waiver program are as follows:
- Resident of Missouri
- Age 21+
- Enrolled in Medicaid
- Serious and complex medical needs which require medical care equivalent to the level of care received in an intermediate care facility (ICF) and must have been eligible for private duty nursing through the HCY program.
- Not enrolled in another waiver

To transfer your current MFAW services to Hyatt Home Health Care Inc., contact your MO Bureau of Special Health Care Needs (BSHCN) Service Coordinator and request transfer of services.

Medically Fragile Adult Waiver (MFAW) program services are authorized through the MO Bureau of Special Health Care Needs (BSHCN).
